Alternative Registration Methods
Patients wishing to register must live within our practice area (view map).
Please call into the surgery to complete and sign the appropriate registration forms. Alternatively you may prefer to download the following forms to complete and sign at home before bringing them into the practice.
- Form GMS1 (PDF) (one to be completed for every person registering at the practice) plus
- Over 16 New Patient Questionnaire (PDF) (for every adult)
- Under 16 New Patient Questionnaire (PDF) (for every child)
- Text Message & Email Consent Form (PDF) (for all patients)
- Patient Consent Form (PDF) (for patients aged 12 and over)

When attending the practice please also bring with you proof of identification e.g. passport or driving licence and a copy of a utility bill.
